UPS Label Instructions

Ottobock have created a label and paid for a collection with UPS on your behalf, we would kindly request that you follow the instructions below:

1. Please ensure the items are packed in a box suitable for transit (preferably the original box that the items were received in).

2. Please print the attachment (this includes 1 x UPS Collection Label & 3 x Commercial Invoices).

3. Attach the UPS Collection Label to the parcel, so that the barcode can be scanned by the UPS driver.

4. Sign & date all 3 copies of the Commercial Invoice, attach these to your parcel in a clear document wallet.

5. If you do not already have a daily collection arranged with UPS you will need to book a collection by calling the free phone number 03457 877877 and following the instructions.

Where prompted, please use the following account details when booking to ensure you are not charged for the collection:
Account Number: 9W3447
Account Postcode: TW20 0LD

UPS will not automatically turn up to collect, you must arrange a time and date with them to collect from you.

Once you have completed these steps UPS will collect your parcel from you at your agreed date and time. Should you require a receipt, one can be obtained from the UPS representative over the phone. You can track your consignment online here.

IMPORTANT NOTE IT IS THE SENDER'S RESPONSIBILITY TO ENSURE THE APPROPRIATE DOCUMENTS ARE USED AND THAT THE CONTENTS OF EACH PARCEL MATCH THE DOCUMENTS. LIABILITY OF COST FOR LOST ITEMS IS WITH THE SENDER. THE UPS LABELS PROVIDED CANNOT BE RE-USED. CONTACT BOCKUK@OTTOBOCK.COM FOR ANY QUERIES.
